Business Operations - Professional Membership Request Form
Purpose:
This form is used to approve all membership requests and/or renewals.

What forms do I need?

- Professional Membership Request Form
- Requisition form (if seeking direct payment to the sponsoring organization)
- Employee Expense Reimbursement Form (if seeking employee reimbursement)
How to complete the Professional Membership Request packet:
To seek approval
- Complete the Professional Membership Request form.
- Gather all required documentation.
- Copy of invoice
- Documentation supporting why membership is being requested.
- Obtain all required signatures.
- Dean/Manager
- Union Committee Chair (if funds for membership are being paid by the Local 1600 Union.)
To seek direct payment to the sponsoring organization
- Complete requisition
- Gather all required documentation.
- Approved Professional Membership Request form (including all signatures)
- Copy of invoice
- Documentation supporting why membership is being requested.
- Enter requisition into PeopleSoft system.
- Attach documents to PeopleSoft requisition
- Submit the completed packet to the Business Office.
- The Business Office will obtain the remaining, required signatures from the Executive Director, President and Vice Chancellor’s Office.
To seek employee reimbursement
- Complete Employee Expense Reimbursement form
- Gather all required documentation.
- Approved Professional Membership Request form (including all signatures)
- Copy of invoice
- Proof of payment
- Original receipt
- Copy of cancelled check
- Credit card receipt or statement
- Obtain all required signatures.
- Dean/Manager
- Submit the completed packet to the Business Office.
- The Business Office will obtain the remaining, required signatures from the Executive Director, President and Vice Chancellor’s Office.
