Chicago Lives Healthy applies to
the following CCC e​mployees who are enrolled in a City Colleges of Chicago medical insurance plan as of January 1, 2014:
                                                          

  • All Local 1600 Full-Time and Part-Time
    Employees (except for security)
    and spouse/domestic partner/civil union spouse    
  • ​Non-Bargained for Employees and spouse/domestic partner/civil union
    spouse        

Chicago Lives Healthy​ is
a long-term wellness program that provides a powerful but flexible approach to
wellness aimed at helping you improve your health and well-being
. Useful information and links can be found on this page to help participants navigate enrollment for the Chicago Lives Healthy program.

Chicago Lives Healthy is a city-wide wellness program designed to help participants improve their health and well-being. The dates and information on this page are accurate for all eligible CCC participants.

The Chicago Lives Healthy program gives you free access to: 

  • Annual biometric screenings
  • Annual Well-Being Assessments (WBA)
  • Annual health check-in call
  • Regular health coaching class
  • Ongoing health activities, programs and classes throughout the year

Your Participation

Participation in Chicago Lives Healthy for the eligible CCC employees listed above is not mandatory. However, you will incur a $50 per non-participant increase in your monthly employee health care contribution for each employee/spouse/domestic partner/civil union spouse who does not meet the program requirements by September 30. This increase will be deducted from your paycheck beginning in the fourth quarter of 2014. You can prevent the ongoing $50 or $100 monthly deduction if you complete any outstanding program requirements prior to the next monthly deduction.

Your personal health information is important and should be kept
confidential. That is why City Colleges of Chicago and Healthways, our wellness
program partner, have a strict policy in place to protect your privacy rights.
All personal health information that you share through the Chicago
Lives Healthy Wellness
Program will remain confidential in full compliance with the Health Insurance
Portability and Accountability Act (HIPAA) of 1996 and will only be used and
disclosed as permitted by law.  Learn

Steps to Enroll

In order to participate in Chicago Lives Healthy, CCC eligible employees must complete the following steps within the given time frames below. Remember, CCC participants have different time frames than other city organizations involved in Chicago Lives Healthy.

                                                                                                 

Step 1: Schedule and attend your annual biometric screening

            All screenings must be completed by May 17.
           The self-scheduler for on-site screenings will be open April 10 - May 16. 
                                                                                                          
The biometric screening is a quick confidential check-up of your height, weight, waist measurement, and finger stick to measure cholesterol, triglycerides and glucose levels. Biometric screenings will take place at each City College of Chicago, as well as the District Office, between May 5, 2014 and May 17, 2014. Both employees and their eligible spouses/domestic partners/civil union spouses may be screened at any available CCC location.
This is an annual event – even if you had a biometric screening last year, you must be screened again in 2014.

The self-scheduler opens on April 10. To schedule your screening, follow the link above to the scheduling portal. Your user ID will be your last name and first initial, with no spaces. Then, enter your date of birth and the group code displayed under the “group code” box. Finally, click "log in."          
                                       

You can also go to your physician’s office to complete your biometric screening. Biometric screenings done at your physician’s office will be subject to your current medical plan benefits. Results of your biometric screening completed by your physician must be submitted on a personalized physician form.  If your physician submits biometric results on any other type of form, the results cannot be accepted and your enrollment Step 1 will be considered incomplete.  Be
sure to obtain a copy of the completed form signed by your physician and a fax
confirmation for your records after it has been submitted.

To obtain your Personalized Physician Form click the red button above and enter your first name, last name, date of birth, gender and zip code to print out your Personalized Physician Form and take it with you to your screening appointment at your physician’s office. You must download your own form, and cannot use a form with anyone else’s name on it.  ​              

       

Step 2: Complete the Well-Being Assessment  

            Complete your Well-Being Assessment by June 30.

The Well-Being Assessment (WBA) is a confidential questionnaire that assesses life and environmental factors that are critical to your health, well-being and your ability to improve both. Fill out this online health questionnaire by June 30.

To take your WBA, follow the link above to the Chicago Lives Healthy participant portal. If you participated in Chicago Lives Healthy last year, you can sign in with your username and password. If you are a new participant, click the orange “Sign Up” button and follow the steps to register. You will then be able to take the WBA.                                                                                                                                              

Health Check-In Call

Once you have completed your biometric screening and your Well-Being Assessment, you will have the option to participate in a check-in call from a certified health professional. The purpose of the 15 minute call is to provide an opportunity for all Chicago Lives Healthy wellness participants to benefit from individualized counseling regarding their health and well-being.

                                               
When you complete your call, you will receive 6 participation points toward your quarterly goal.                                                                                                        

Step 3: Quarterly Wellness Activities Beginning in July 2014


Beginning July 2014, all participants in the Chicago Lives Healthy program will be required to earn 12 new points each quarter by completing wellness program activities.

While Healthways may provide your employer with collective data about its population as a whole, all individual health information is withheld from your employer.
