Overview of BCBS EyeMed vision care benefits:

Vision Plan

The purpose of City Colleges of Chicago’s vision plan is to provide affordable protection from large out-of-pocket vision expenses and encourage
preventive care. CCC pays approximately 85%of your monthly cost for vision plan coverage. The Blue Cross Blue Shield (BCBS) Vision Care
offers you flexibility in choosing your vision provider. You may choose between a BCBS provider or an out-of-network provider. Benefits are better
if you select a BCBS in-network provider. The plan benefits include examinations and lenses every 12 months and frames every 24 months. There
is an individual $10 copayment each calendar year for all covered services.
