Instructions on How to Receive your 2025 Form 1095-C.

Each year, employers must provide eligible employees with IRS Form 1095-C, which shows information about health coverage offered to you. Under the Employer Reporting Improvement Act of 2024 employers no longer have to automatically send Form 1095-C to individuals. Employers may post a notice informing individuals they may request a copy of the statement.

You can choose to receive this form on paper or electronically. Receiving it electronically is optional.
This form lets you give permission to receive your Form 1095-C online instead of by mail.
