City Colleges of Chicago is committed to fully complying

Requesting
Reasonable Accommodation
Employees or applicants should make any and all requests for
reasonable accommodations to the Equal Opportunity (“EEO”) Office. It is the responsibility of the employee or
applicant to inform City Colleges of Chicago that an accommodation is
needed. Employees must complete and
return the ADA Reasonable Accommodation Request Form AND the Authorization for Release of Information Form (available in Forms section at right) to the EEO Office, along with any relevant
documentation.
The
Reasonable Accommodation Process
Once the forms for a reasonable accommodation are received by
the EEO Office, the employee will be notified in writing that the EEO Office is
in receipt of the request. The EEO
Office will review the information to make a determination. If, after the employee has provided
documentation, the nature or extent of the employee’s disability or the
relevancy of the requested accommodation is unclear, the employees may be asked
to meet with the EEO Office.
If the review of the employee’s information and supporting
documentation indicates that a reasonable accommodation is warranted, the EEO
Office will consult with the employee to establish an effective and feasible
accommodation that will enable the employee to perform the essential functions
of the position. Reasonable
accommodations will be made in accordance with the nature of the disability and
the nature of the employee’s essential work duties.
Within a reasonable period of time, the employee will be
informed in writing of the final action taken on the accommodation request,
including what, if any, accommodation will be provided. The EEO Office will
record the final action taken by City Colleges of Chicago.
Confidentiality
All information regarding an employee’s or applicant’s
disability and the reasonable accommodation request is confidential, with the exception
of the employee’s supervisor(s) and other personnel who need to be informed in
order to assess requests for, implement, or monitor accommodations. Any documentation will be kept confidential
and separate from the employee’s departmental personnel file.
